We, the undersigned parties, hereby agree to submit to arbitration<\/em>, which shall be <\/em>administered by Lextar Arbitration Services under its Commercial Arbitration Rules<\/em>,<\/em> the <\/em>disputes that have arisen between the parties hereto.<\/em><\/p>\n\n\n\n

We further agree that the above controversy be submitted to <\/em>the arbitral tribunal consisting of <\/em>\u2610one <\/em>\u2610three arbitrator(s). <\/em>In the case of a 3-member arbitral tribunal, each party may appoint its own arbitrator with the third one to be appointed by Lextar Arbitration Services to serve as the presiding arbitrator. In the case of a sole arbitrator tribunal, we agree that the sole arbitrator be appointed by Lextar Arbitration Services if we fail to agree on the sole arbitrator within 5 days after the claimant<\/em>\u2019<\/em>s request for arbitration is accepted.<\/em><\/p>\n\n\n\n

We further agree that we will faithfully observe this <\/em>Supplemental Arbitration A<\/em>greement and the rules, that we will abide by and perform any award rendered by the arbitrator(s<\/em>)<\/em>.<\/em>
